We are excited to open up our office to care for your visual needs. To provide a safe environment for our patients and staff, we are adhering to strict CDC cleaning protocols and following all recommended guidelines from our local officials.
Therefore,
an approved mask (covering both nose and mouth) is required throughout the entire visit.
Masks with vents are not allowed. Masks are available at our office for an additional fee.

We are also limiting the number of people in our office by accepting appointments only (to help us enforce this policy, we ask that patients come alone or accompanied with a legal guardian if the patient is a minor). Please note that anyone with the following may be rescheduled:
- Been exposed to anyone with COVID in the last 30 days,
- Have experienced any symptoms such as fever, cough, loss of taste and/or smell in the last 30 days
When making an appointment, please have both vision and medical insurance info ready, if applicable.
New patients: please fill out and bring the appropriate
forms to your appointment.
